EFT MEMBERSHIP AGREEMENT

If for any reason the payment is declined, I will be charged a fee of _______________ (subject to change as the bank fees change), and my account will be frozen until the payment and fee is received.

I may cancel at any time with 20 days written notice (proof of notice required). The start-up fee is non-refundable.
I may freeze my account with 20 days prior notice at the rate of $5 per month.
The monthly rate is guaranteed for 1 year, after that, if the rate increases Ship Shape Tanning must inform me within 30 days, in person, by phone, or by letter to the address on file. If I have not kept my file up to date, Ship Shape Tanning may consider me informed of the increase.
All Platinum Tanning is included, I may upgrade to any upgrade bed or booth for 50% off. Platinum tanning included in my membership is on a first come first served basis.
I may purchase any product for 10% off - even sale items!
Memberships may not be shared and are limited to the hours we are open and the laws of the State of Washington.
Although we advocate tanning in moderation and highly recommend that you skip at least one day in between tans, you are legally entitled to tan once per day at this time.
